55%-100% subsidized memberships to children ages 4-7 from low-income families (as defined by Statistics Canada)

How do I qualify?

1. Do you have a child between the ages of 4-7?

2. Does your family income fall in the below Low Income Cut Off chart (as per Statistics Canada)?

If YES to BOTH questions…you qualify!

HOW DO I APPLY?

Apply TODAY in 3 easy steps!

STEP #1: Proof of Income

Gather ONE of the following documents to provide proof of income:

a) 3 consecutive pay stubs (within the last two months)

b) Prior year T4

c) Income assessment (Notice of assessment)

d) Universal Childcare Benefit Assessment

e) Ontario Works stub

f) Unemployment statement showing benefits

These documents can be found on your Canadian Revenue Agency Account

a) When filling out the application ensure that the third page of the application reflects the following information:

100% Subsidized 6 Weeks Slam Dunkers Membership (4-5 year olds)

6 week Slam Dunkers Membership

50% Subsidized 3 Months Slam Dunkers OR JR. NBA Membership

12 Week Slam Dunkers OR Jr. NBA Membership

6 Months Slam Dunkers Membership (4-5 year olds)

6 Months Slam Dunkers Membership

STEP #3 Approval

Forward approval email from Jumpstart Regional Manager to team@littledribblers.ca

- May take 3-4 weeks to receive approval email
- If enrolling in any other program other than the 6 Week Slam Dunkers program, the balance must be paid to secure your spot
